Reassurance: Medically Unexplained Physical Symptoms
نویسنده
چکیده
Bill Anderson Escobar et al.’s discussion of medically unexplained physical symptoms is useful, and could trigger a renewal of how the medical profession works [1]. We are in transition at the present, and what we have been trained for is less and less relevant. This disjunction between our training and practise has given rise to a generation of unhappy doctors. The traditional “doctor” dealt with symptoms and illnesses for which there was no useful explanation and became expert in managing these. We have completed a century or more of science and pathology and have trained doctors to derive their interventions from an (often cursory) consideration of the patient’s illness and the application of reason within this scientifi c framework, in the unshakable belief that this strategy gave the best and at times the only possibility of assisting the patient in a meaningful way. The practical effects of such practices were rarely if ever examined. This approach had many successes but lots of failures, and we have moved on. We now have built upon the science a growing evidence base which to date principally addresses disease states (pathological processes) rather than illnesses (patients’ experiences). There is however no reason why evidence-based approaches cannot be applied to the management of symptoms and illness as well as disease states. As we develop that evidence base, the nature of clinicians can change. We will not require all “doctors,” for want of a better word, to be comprehensively trained in bioscience. There will be more need for them to deploy skills in implementing well-developed, evidence-based interventions and fi tting them to the illness the patient is experiencing. Such “doctors” will deliver most care and pay better attention to the patient’s symptoms and illness than we do today. We will of course require another different sort of “doctor” who will do the research and the sifting to develop the evidence base upon which therapies will become universally dependent.

Why Reassurance Fails in Patients with Unexplained Symptoms—An Experimental Investigation of Remembered Probabilities
BACKGROUND Providing reassurance is one of physicians' most frequently used verbal interventions. However, medical reassurance can fail or even have negative effects. This is frequently the case in patients with medically unexplained symptoms. It is hypothesized that these patients are more likely than patients from other groups to incorrectly recall the likelihoods of medical explanations prov...

Any terrorist attack using weapons of mass destruction will result in substantial psychological trauma and stress. Primary care and emergency clinics will likely see patients who have stress-related emotional or physical symptoms, or exacerbations of preexisting health concerns. Significant psychological and behavioral reactions to an attack with weapons of mass destruction are certain, include...
متن کاملNormalisation of unexplained symptoms by general practitioners: a functional typology.
BACKGROUND Patients often present in primary care with physical symptoms that doctors cannot readily explain. The process of reassuring these patients is challenging, complex and poorly understood. AIM To construct a typology of general practitioners' (GPs') normalising explanations, based on their effect on the process and outcome of consultations involving patients with medically unexplaine...
متن کاملMedically unexplained symptoms--GPs' attitudes towards their cause and management.
BACKGROUND Medically unexplained physical symptoms present one of the most common problems in modern medical practice but often prove difficult to manage. The central position of the GP in the care of patients with medically unexplained symptoms has been emphasized repeatedly, but little is known about the attitudes of GPs to this role. Understanding how GPs view these patients may inform the d...
متن کاملMedically unexplained physical symptoms in emergency medicine.
Medically unexplained physical symptoms (MUPS) are physical symptoms for which no relevant organic pathology can be found. Patients with MUPS commonly present to the emergency department (ED) but are rarely considered in emergency medicine teaching or literature. Management of these patients is frequently more challenging than where there is an obvious organic pathology. This review provides th...
متن کاملThe resource utilisation of medically unexplained physical symptoms
OBJECTIVES As patients with medically unexplained physical symptoms may present frequently to hospital settings and receive potentially unnecessary investigations and treatments, we aimed to assess the frequency and type of medically unexplained physical symptoms presentations to clinical services and estimate the associated direct healthcare costs. METHODS This study was undertaken at the la...
